Service Coordinator - Full-Time

The Service Coordinator will work in the Outreach and Access to Recovery Program for BIPOC (Black, Indigenous and People of Color). The overall goal of the program is to address the disparities experienced in BIPOC communities regarding substance abuse including higher rates of overdose, lower likelihood of being identified as needing care, and lower access to needed services.

This full-time position with Macedonia Family and Community Enrichment Center will work with the team to

increase awareness regarding the risks of substance abuse and overdose. The Service Coordinator works directly with community members identified as needing substance abuse intervention and/or treatment and assists them in assessing needed resources. The Coordinator arranges and facilitates referrals to needed services and helps eliminate any barriers to access. The Coordinator provides support to individuals as they engage in intervention and treatment, encouraging them to complete recommended treatment. The Coordinator works with familial supports to provide information and resources to address needs and support the family in adjusting to changes required by the recovery process

What Will I do?

​

  • Engage community members and their families to increase awareness of substance abuse risks and to assess services when needed.

  • Facilitate group activities at community events that provide substance abuse knowledge and harm reduction skills that mitigate risk.

  • Interview and elicit pertinent information from participants.

  • Make referrals for screening and assessment when needed.

  • Arrange for treatment admissions; follow up and support consumers and their families through the treatment process.

  • Provide case management for families involved in the program by assisting them in setting and achieving goals.

  • Conduct home visits and visits to residential treatment facilities to support consumers and their families.

  • Document activities and client data in client services database, compiling any needed report.

Experience/Minimum Requirements:

​

  • Bachelor’s degree in social work, psychology, sociology, counseling, education, or other health related field

  • Knowledge and/or experience working with substance abuse concerns

  • Knowledge about harm reduction and treatment resources for substance abuse

  • Ability to work well with people of diverse social, cultural, and economic backgrounds

  • Proficiency with Microsoft Office Suite

  • Excellent communication ability

  • Organizational skills, along with detail-orientation

  • Act 33, 34 and FBI clearances required

  • Valid PA driver’s license, clean driving record, insurance and registration required

 

SALARY: $45,000/annual

All positions require satisfactory results of a background check and drug screen, as well as Act 33/34 and FBI fingerprint clearances. All employees will be subject to a probationary period of no less than 180 calendar days.
