Ryan White & Case Management Supervisor - Full-Time

The Ryan White and Case Management Supervisor is responsible for overseeing the operations of the Ryan White Services Unit and Adult Case management for senior citizens previously incarcerated. These programs include HIV Case Management, HIV Education and HIV testing, linkage of referrals and overall support to senior citizens.

ESSENTIAL RESPONSIBILITIES:

​

  • Supervises and provides training, bi-weekly supervision and mentoring support for Case Managers and Program Assistants

  • Provides guidance with cases along with appropriate feedback and recommendations

  • Assures staff members adhere to all of the models used in service delivery, i.e., provide measurable and clinical feedback

  • Resolves complaints made by clients and stakeholders in a timely manner

  • Discusses and resolves any job performance problems with employees to identify causes and issues

  • Assists with facilitating unit meetings and ongoing training for staff

  • Attends community meetings with partner agencies who work in the field of HIV/AIDS

  • Communicates with collateral contacts and stakeholders

  • Provides weekly/monthly quality assurance checks on files and unit paperwork (meets all deadlines)

  • Establishes calendar of events to meet contractual agreements

  • Provides assistance with service delivery to Case Managers and Program Assistants as needed

  • Acts as the liaison as the agency continues to build and promote partnerships and collaborations with service providers and other entities

  • Manages the electronic case management system required by Pennsylvania Department of Health and the County Area Agency on Aging

  • Monitors activities and prepares correlating monthly reports

  • Participates in leadership & management team meetings

  • Assures quality in programming, data entry and reporting

  • Supervise case management support and linkage to referrals to senior citizens previously incarcerated

EXPERIENCE/MINIMUM REQUIREMENTS:

​

  • Master’s Degree in social science, education, psychology or related field; MSW a plus

  • Minimum 5 years’ human service experience required

  • Minimum 3 years’ program management experience that includes experience with staff supervision

  • Demonstrated ability to provide clinical supervision

  • Demonstrated knowledge about the strengths, needs and barriers for people living with HIV and AIDS, and senior citizens

  • Demonstrated knowledge of Ryan White and other funding sources for the HIV population, and senior citizens a plus

  • Life/work experience must demonstrate commitment to children and families and community agencies

  • Familiarity with the Hill District and surrounding area resources and community service providers

  • Ability to work well with people of diverse social, cultural and economic backgrounds

  • Strong strategic planning ability to include written/verbal communication skills

  • Ability to work flexible shifts to include the attendance of evenings meetings as required

  • Proficiency with Microsoft Office Suite

  • Excellent communication and organizational skills, along with detail-orientation

  • Act 33, 34 and FBI clearances required

  • Valid PA driver’s license, clean driving record, insurance and registration required

 

SALARY: $50,000 for candidates with full benefits package
