Intake Coordinator - Full-Time
JOB SUMMARY:
The Intake Program Assistant is responsible for overseeing the Lifeline referral process for the agency and serves as the initial contact with referral sources for the program. Major responsibilities are to conduct all administrative/intake duties for Lifeline including interacting with referral data management systems. The Intake Program Assistant will cooperate with facilitators, coordinators, caseworkers, families, service providers and administrators. This position will work closely with the Family Engagement Services Supervisors, Family Coordinators and interns to assure that referral and case assignment tracking are accurate. A commitment to maintaining excellence in all phases of the project is essential.
RESPONSIBILTIES:

Tracks new referrals for Lifeline and FACT both in internal and external systems

ï‚·Interacts with public and staff professionally, developing and maintaining the knowledge to respond to questions about the program and resources

ï‚·Initiates contact and identifies clients’ needs for new Family Engagement Services upon request (Lifeline, FACT, or other identified programming)

ï‚·Provides direct/indirect communication including initiating and responding to phone calls, meetings and sending electronic correspondence to clients

ï‚·Inspects documents, forms, records and other materials for accuracy and completeness; process a variety of forms and documents according to established procedures; assure conformance to established guidelines and standards

ï‚·Provides administrative support by completing data entry into agency database system (Apricot), tracking systems and/or electronic document storage system Docuware

ï‚·Assists and schedules coordination of Lunchbox deliveries to clients
-
ï‚·Provides linkage to resources for families with a focus on reaching self-sufficiency
-
Provides information and answer questions about Lifeline from other departments, referral sources and the general public
-
Submits regular program data and reports to Program Manager

Carries a small caseload depending upon the business needs of the agency

Serves as back-up to the administrative team for front office coverage

Maintains a high level of ethical conduct regarding confidentiality and professionalism

Completes and maintains written documentation

Participates in ongoing trainings and remains knowledgeable in areas of expertise.

Attends internal agency meetings as appropriate and meets regularly with the Program Manager

Assists with agency special projects upon request

Represents the agency at stakeholder meetings upon request

REQUIREMENTS:
-
2 years' experience in non-profit office administration or equivalent combination of education and experience
-
1-3 years’ case management working with children and families
-
Experience and thorough knowledge of methods and procedures used in determining eligibility for social
-
services
-
Excellent organizational skills with the ability to manage multiple projects
-
Proficiency with computer systems and excellent grammar and composition skills
-
The ability to function in a fast-paced working environment.

Reliable Internet service, computer and phone equipment.

SALARY: $40,600 - $43,700

All positions require satisfactory results of a background check and drug screen, as well as Act 33/34 and FBI fingerprint clearances. All employees will be subject to a probationary period of no less than 180 calendar days.
