Community Engagement Case Manager 

The Community Engagement Case Manager will provide comprehensive nonmedical case management to the target population for The Wise Neighbor project, senior citizens residing in the Hill District, Homewood and East Liberty neighborhoods of Pittsburgh. This role with Macedonia Family and Community Enrichment Center is a professional position working in the development and coordination of aging services, public health education, health promotion, and community outreach activities.
 

This position provides basic case management to older persons, supports aging in place, and positively influences social determinants of health. A commitment to maintaining excellence in all phases of the project is essential, with the ability to collaborate with senior center staff, administrators, and community partners. Utilizing sensitivity, awareness, and skills associated with working with individuals of all ages and backgrounds, the Community Engagement Case Manager will provide consumers with information and referrals and work toward positive outcome resolutions.

What Will I do?

  • Interview and elicit pertinent information from community members regarding the need for assistance with housing, healthcare, food, or other resources to support basic needs.

  • Conduct home visits to senior living facilities, residential homes, and other identified locations to engage with older persons over the age of 60.

  • Conduct comprehensive assessments to identify required services.

  • Interpret rules and regulations regarding eligibility for other programs and linkages to referrals.

  • Support clients in short and long-term goals developing individual service plans as required.

  • Provide referrals and information to clients.

  • Provide follow-up on actions steps, goals, and plans by scheduling routine reviews/reassessments

  • Coordinate services with other social service providers and healthcare entities as appropriate.

  • Maintain accurate and timely case documentation.

  • Act as a liaison with community organizations and Advisory Council as it relates to supporting senior citizens.

  • Participate in ongoing trainings and remain knowledgeable in areas of expertise (aging services, social determinants of health, healthcare).

  • Assist with compiling data to prepare records and reports.

  • May work evenings and weekends with advance notification.

  • Assist in city-wide events.

 

What will make you a great fit (the ideal candidate for this role has a unique blend of the following):

  • Bachelor’s degree in social work, public/business administration, education, health or related field, required

  • 2-3 years’ case management experience

  • Demonstrated knowledge about the strengths, needs and barriers for senior citizens

  • Knowledge of community resources for appropriate referrals

  • Demonstrates ability to use motivational interviewing techniques

  • Ability to work well with people of diverse social, cultural, and economic backgrounds

  • Proficiency with Microsoft Office Suite

  • Excellent communication and organizational skills, along with detail-orientation

  • Act 33, 34 and FBI clearances needed

  • Valid PA driver’s license, clean driving record, insurance and registration required

 

SALARY:

$41,300 to $43,300 - commensurate with experience and education background

All positions require satisfactory results of a background check and drug screen, as well as Act 33/34 and FBI fingerprint clearances. All employees will be subject to a probationary period of no less than 180 calendar days.
